NOTE: Registration fee is $150 for the 2008 season. Partial season registration is $50 per three race events payable prior to first event. Partial season registration holders are subject to number re-assigment.

By signing below I hereby release and hold harmless the ShortTrackTrucks.com Challenge Series (wholly owned by LOOKFAST! Racing, Inc.), its agents, officials, and associated entities from all liability and damages associated with competition in events sanctioned by ShortTrackTrucks.com Challenge Series/LOOKFAST! Racing, Inc. I understand that this registration is for issuance of a competitors license, which is not an insurance policy. I further agree to purchase a pit pass and competitor insurance before entering the pit area or engaging in competition and in consideration of the foregoing, I do hereby release, remise and forever discharge ShortTrackTrucks.com Challenge Series/LOOKFAST! Racing, and all officers, directors, agents, employees, the sponsors and manufacturers of all racing equipment upon the premises, from all liability claims, actions and possible causes of action whatsoever that may accrue to me or my heirs, next of kin and personal representatives from every and any loss, damage and injury (including death) that may be sustained by my person and/or property while in about, en route into and out of premises of a sanctioned racing event or any premises where LOOKFAST! Racing/ShortTrackTrucks.com Challenge Series events are presented.

 

I agree to abide by the series rulebook and its provisions as interpreted by the ShortTrackTrucks.com Challenge Series officials. In consideration of being granted a license to participate in ShortTrackTrucks.com Challenge Series events, the undersigned hereby, agrees they have read the rules and will abide by all ShortTrackTrucks.com Challenge Series rules, regulations and amendments, and officials decisions, during the calendar year of this license period.

 

I grant the ShortTrackTrucks.com Challenge Series the right to use my race team and driver's likeness, image, and name in any and all Series promotional material. In consideration of awards for ShortTrackTrucks.com Challenge Series events and the acceptance of this license application by the ShortTrackTrucks.com Challenge Series, the licensee assigns use of their name(s) and pictures of themselves and their race car presented for competition at the site of business of the ShortTrackTrucks.com Challenge Series, to the ShortTrackTrucks.com Challenge Series for their use in any medium or material for the purpose of promoting, advertising, recording or reporting and reproduction of same for sale for any ShortTrackTrucks.com Challenge Series purpose.

 

I have read and fully understand this waiver and release of liability and indemnity agreement and I know and understand my signature on this application form operates as a complete release of LOOKFAST! Racing/ShortTrackTrucks.com Challenge Series together with its officers, directors, officials, representatives, agents and employees from any and all liability, including negligence, and I freely and willingly consent to this waiver and release of liability and indemnity agreement.
